Consent Forms…Whats included?

Yesterday I signed my consent forms online for my clinic. I wanted to get them done before I start my tracking cycle which should potentially be due to start within the next few days or so.

Normally the forms would be signed paper copies at the clinic – however with everything that is going off they’re all online forms to read and sign.

I thought I would give you an insight into the forms that I’ve just signed for my Fertility Journey and what they are for / include.

HFEA CD Form
This is a consent to disclose information form from the HFEA or the Human Fertilisation and Embryology Authority for those of you who don’t like abbreviations. This is about sharing your information with other medical professionals and personel including but not limited to your GP. It also asks whether or not your details can be used for any form of research such as contact or non-contact including non-indentifying information.

Patient Identification Information
This is as simple as it sounds – name, contact numbers, date of birth and upload an image!

Pre Treatment Consultation Record
Basic details that you would expect to fill in.
Date of birth, Address, Maiden name if applicable, ethnic origin and occupation.
It then goes on to include questions such as how long if applicatble you’ve been trying to get pregnant, relevant medical & surgical history. Any treatment on your cervix, allergies, alcohol and smoking information, tickbox of other items such as blood pressure, asthma, diabetes etc, what regular medication you take, are you taking folic acid, is there someone to take you home and look after you after egg retrieval, what countries you’ve visited in the last 12 months…

IVF Pre Treatment Checklist and Information
This explains everything for IVF & ICSI and is split into sections which includes consenting to all the following too.

Zika Virus Information
This is about where Zika is and how it can affect pregnancies etc

Corona Virus Information
This is about Coronavirus, and how the HFEA and clinics are working together and statements about us trying to keep ourselves out of harms way and not putting ourselves in situations where we may come into contact with coronavirus and follow guidelines.

HFEA Welfare of the Child Form
This form is as it states, its a history form to see if any child that you have as a result of fertility treatment may come to some form of harm.

HFEA WT Form
This form is the womans form from the HFEA for consent to treatment of storage of Egg and Embryo’s, how long they’ll keep them on file, and also using eggs and embryo’s for training purposes after I no longer “need” them, but also what would happen to them in my death or mental incapacity.

As you can see there is some things that you may not have even considered, or some things that you may need to take time to consider before you sign your consents.

There are partner information too for some of the forms. Men have to sign different forms too – but obviously thats irrelevant for my solo mum journey but just so people are aware 🙂
